Description
Allopurinol is a xanthine oxidase inhibitor that prevents uric acid formation from xanthine and hypoxanthine. It lowers uric acid and its salts concentration in bodily fluids, helps dissolve existing urate deposits, and prevents new ones from forming in tissues and kidneys. It's prescribed for hyperuricemia-related conditions including gout (primary and secondary), urate kidney stones, and elevated uric acid levels due to nucleoprotein breakdown in diseases like leukemia.
Dosage and direction
The daily dose ranges from 100-800 mg based on condition. Start with 100 mg, gradually increasing if needed. For acute gout and tophi (uric acid crystals around joints), 300-400 mg daily is recommended. Doses exceeding 300 mg should be split into multiple intakes. Take with food to prevent stomach irritation. Avoid high vitamin C doses. Maintain adequate fluid intake to prevent kidney stones. Limit alcohol and purine-rich foods like organ meats, sardines, and lentils.
Precautions
Allopurinol requires months of continuous use for symptom improvement. Initial treatment may trigger gout attacks even with normal uric acid levels; these will decrease over time. Discontinue and consult a doctor if allergic reactions occur, as severe skin conditions, liver damage, or blood vessel inflammation may develop. Patients with diabetes or kidney issues need dose adjustments.
Contraindications
Allopurinol is contraindicated in patients hypersensitive to the drug, those with liver/kidney disorders, primary hemochromatosis, asymptomatic hyperuricemia, acute gout, pregnant or breastfeeding women, and children under 14 (except for leukemia, tumors, or enzyme disorders cases). Use cautiously in renal/heart failure, diabetes, or hypertension.
Possible side effects
Common side effects include diarrhea, nausea, allergic reactions (rash, itching), and drowsiness. Stop medication immediately if rash, painful urination, eye irritation, or facial swelling occur, as these may indicate severe allergic reaction. Rare cases may involve nerve, kidney, or bone marrow damage, or potentially fatal liver toxicity. Liver toxicity symptoms include appetite loss and itching, with higher risk in kidney impairment patients.
Drug interactions
Allopurinol increases blood levels of mercaptopurine (Purinethol) and azathioprine (Imuran), requiring dose reduction. Combining with penicillins raises rash risk.
Missed dose
Take the missed dose when remembered unless close to the next scheduled dose. In that case, skip it and resume normal schedule. Never exceed recommended dosage.
