Description

Trihexyphenidyl is an anticholinergic medication primarily used to treat symptoms of Parkinson's disease and drug-induced movement disorders. It works by blocking acetylcholine receptors in the nervous system, helping to restore the balance between dopamine and acetylcholine in the brain. This action helps control tremors, muscle stiffness, and excessive salivation associated with Parkinson's disease.

The medication is available in tablet and liquid forms, typically taken 1-4 times daily as prescribed. Common side effects include dry mouth (occurring in about 60% of patients), blurred vision (30-40%), constipation (20-30%), and urinary retention (15-20%). These effects result from the drug's anticholinergic properties and often diminish with continued use.

More serious but less frequent adverse effects require immediate medical attention:

Allergic reactions may present with facial swelling, difficulty breathing, or skin rashes. The drug can impair temperature regulation, potentially causing heat stroke in warm environments. Neuroleptic malignant syndrome, though rare, manifests with high fever, muscle rigidity, and altered mental status. Elderly patients are particularly susceptible to confusion, hallucinations, and memory problems.

Trihexyphenidyl is contraindicated in patients with narrow-angle glaucoma, bowel obstructions, or severe urinary retention. Caution is advised for those with cardiovascular disease, hyperthyroidism, or prostatic hypertrophy. The medication may worsen tardive dyskinesia and should be used carefully in psychiatric conditions.

Important drug interactions include:

Central nervous system depressants like alcohol and opioids may enhance sedation. MAO inhibitors and tricyclic antidepressants can potentiate anticholinergic effects. The medication may decrease the effectiveness of antipsychotic drugs while increasing digoxin absorption.

Special precautions:

Dosage should be gradually adjusted over 2-3 weeks to minimize side effects. Abrupt discontinuation may cause withdrawal symptoms. Patients should maintain adequate hydration and avoid overheating. Regular monitoring of intraocular pressure is recommended for those at risk of glaucoma.

Storage requirements include keeping the medication at room temperature (15-30°C) in tightly closed containers. Liquid formulations should be measured with proper dosing devices, not household spoons.

For missed doses, patients should take the medication as soon as remembered unless close to the next scheduled dose. Double doses should never be taken. In case of suspected overdose (symptoms may include severe confusion, seizures, or coma), immediate medical attention is required.
